CSUDH Self-Assessment Screening Tool

The self-assessment screening tool must be completed by anyone coming to campus. Individuals must complete this screening each day prior to coming on campus, or at the beginning of each day if they reside in university housing.

Notice to Individuals Who Have Been Vaccinated:

If you are feeling ill after receiving the vaccine, please stay home as you would for any other illness. Please do not call the hotline for symptoms related to being vaccinated. Symptoms from vaccinations typically do not last longer than 48 hours, if you have persisting symptoms please contact your medical provider.
